To start the pre-operative surgical assessment process, Tinley Woods Surgery Center requests that you fill out your medical history online with HST Pathways Pre Assessment. It is recommended that you enter your online medical history as soon as you receive your personalized link. This link will be sent via text or email once your surgery is scheduled. After you complete your online history, the pre-op assessment nurses will be able to access this information so they can organize and document your medical history to prepare you for surgery.

Please make sure to have the following information available:

• A list of all prior surgical procedures.
• A list of all of your current medications including dosage and frequency of use.
• Your surgeon’s name and your procedure.
• The name, address and phone number of your primary physician as well as the names and contact information for any specialists you see on a regular basis.
